In Times of Bereavement
If death takes place at home, you will need to do the following:
- Please telephone the Surgery, the GP will visit to confirm the death.
- You will then need to telephone a funeral director.
- The doctor will complete a Medical Death Certificate for you to collect.
- You will then need to take this to the local Registrars Office in which the death took place, (you will also to take with you the deceased’s Birth Certificate and Medical Card, if available). You can also register by declaration at a convenient Registrars Office, however certificates will have to be posted to you after a few days.
- A Green coloured certificate will be given to you by the Registrar to pass to your funeral director who will look after all of the necessary arrangements for the funeral. A white notification certificate for the DSS will also be issued by the Registrar, they will also ask you how many Certified Copies you will need for dealing with the finances of the deceased (you will be charged a fee per copy).
If death takes place in hospital:
- You will need to telephone a funeral director who will assist you with all of the necessary funeral arrangements.
- You should collect the certificate from the hospital.
- You will then need to take this to the local Registrars Office in which the death took place, (you will also to take with you the deceased’s Birth Certificate and Medical Card, if available). You can also register by declaration at a convenient Registrars Office, however certificates will have to be posted to you after a few days.
- A Green coloured certificate will be given to you by the Registrar to pass to your funeral director. If cremation is preferred.
Additional certification is required for cremation, however the funeral director will arrange this with the Surgery.
